Complete the Patient Information Sheet and Assignment of Insurance Benefits (AOB) form. This short form allows us to verify your insurance coverage for you with no further obligation. (You will need Adobe Acrobat Reader to view the AOB form.)
Step 2
Submit this information to Medtronic Diabetes by either faxing it to 1-800-433-9867 or by mailing it to 18000 Devonshire Street, Northridge, California 91325-1219, Attention: Pump Therapy Consultants.
Step 3
As soon as we receive your information about your insurance coverage, we will contact you to discuss your benefits and options as well as how to proceed.
We do the legwork for you.
We will assist you every step along the way, from getting a prescription for an insulin pump to obtaining the letter of medical necessity from your healthcare provider.
